Histoplasmosis is a fungal infection caused by Histoplasma capsulatum. The infection primarily affects the lungs and can range from mild to severe, with symptoms including fever, malaise, headache, and weakness. In severe cases, acute respiratory distress syndrome can occur.
The first line treatment for histoplasmosis depends on the severity of the infection. For most patients with acute pulmonary histoplasmosis, no treatment is necessary as the infection typically resolves on its own within 4 to 6 weeks. However, if symptoms persist for longer periods or if the patient is severely ill, antifungal therapy may be required.
For severe cases of acute pulmonary histoplasmosis or disseminated histoplasmosis, amphotericin B is the preferred initial treatment. It is administered intravenously and is typically used for a few weeks followed by maintenance therapy with an azole antifungal drug such as itraconazole.
Itraconazole is the azole of choice for the treatment of mild to moderate histoplasmosis. It is taken orally and is continued for a period of 6 to 12 months. Fluconazole is another option, but it is less effective than itraconazole and is generally considered a second-line agent.
For chronic forms of histoplasmosis, including chronic cavitary pulmonary histoplasmosis and granulomatous mediastinitis, treatment with itraconazole is recommended for a longer duration, ranging from 12 to 24 months.
In cases of mediastinal fibrosis or pericarditis associated with acute pulmonary histoplasmosis, antifungal therapy is not necessary and treatment with nonsteroidal anti-inflammatory agents or corticosteroids may be sufficient.
In severe cases of disseminated histoplasmosis, initial treatment with amphotericin B is recommended, followed by a switch to itraconazole once the patient begins to show improvement.
The length and type of treatment depend on the severity of the infection and the immune status of the patient. Close monitoring and follow-up are necessary to ensure proper response to therapy.
